Patient died at Whittington Hospital after medication was ‘suspended in error’

A PATIENT died after their medication was wrongly stopped in hospital, board papers say.
The Whittington Hospital NHS trust said the death was “potentially avoidable” and that the man had a “large pulmonary embolism”. His prophylactic medication was “suspended in error”, according to this month’s report to the NHS trust board. The documents added: “This was investigated as a serious incident [SI] and the family of the patient have been kept fully informed.”
The trust, under government regulations, is required to publish details of what are formally described as “never events” or “serious incidents” in its monthly board papers.
